Friends and Family Terms and Conditions

1. One Voucher Per Patient
   Each eligible patient may receive only one Friends and Family voucher, regardless of the number of surgeries they undergo.
2. Eligibility
   Vouchers are only issued for completed surgeries. Surgery must be fully completed before a voucher is sent.
3. Surgery Type
   1. Only self-pay or insured surgery qualify.
   2. Excludes surgery financed or via a payment plan.
   3. Surgeries offered free of charge (FOC) are not eligible.
   4. Injections and YAG laser procedures do not qualify.
4. Insurance Restriction
   The Friends and Family discount only applies to self-pay surgeries. It cannot be used for surgeries covered by insurance.
5. Voucher Issuance
   Vouchers are typically sent within 4 weeks of the completed surgery date.
